| Engineer | Form: (02) 400696387 10/10/2014 | 1)Operator shall comply with the most current revision of the Northwest Colorado Notification Policy. See attached notice.
2)Operator shall comply with the most current revision of the Garfield County Rulison Field Notice to Operators, with the following exception: all field notice requirements specified in this notice to operators are superseded by the requirements of the most current revision of the Northwest Colorado Notification Policy (see condition of approval #1). See attached notice.
3)Operator shall provide cement coverage from the production casing shoe (4+1/2" FIRST STRING) to a minimum of 200' above all Mesa Verde Group (and Ohio Creek Formation, if present) oil, gas, and water-bearing sandstone and coalbed formations. Verify production casing cement coverage with a cement bond log.
4)Comply with all provisions of the June 12, 2008 Notice to Operators drilling wells within ¾ mile of the rim of the Roan Plateau in Garfield County – Pit Design, Construction and Monitoring Requirements. See attached notice. |
| Permit | Form: (02) 400696387 11/11/2014 | Open hole resistivity and gamma logs shall be run to describe the stratigraphy of the entire well bore and to adequately verify the setting depth of surface casing and aquifer coverage. On a multi-well pad, these open hole logs are only required on one of the first wells drilled on the pad and the Drilling Completion Report - Form 5 for every well on the pad shall identify which well was logged.

| UIC | Form: (04) 400753373 02/13/2015 | 1. Injection is not authorized until approval of Forms 31 and 33.
2. Prior approval of Form 4 is required for step rate and injectivity tests.
3. A Transient Injectivity Test is required to determine the Maximum Daily Injection Rate in barrels per day (bbls/day).
4. A Transient Injectivity Test or a Step Rate Test is required to determine the Maximum Injection Pressure in pounds per square inch (psi).
5. Retrieve water sample(s) from injection zone(s) before stimulating formation. Samples must be analyzed for Total Dissolved Solids at a minimum.
6. For all new drill Underground Injection Control wells a suite of open-hole Resistivity/Gamma Ray and Density/Neutron logs is required from Surface Casing shoe to TD. A PDF, TIFF, or PDS visual image and a LAS or DILS file version of each log is required.
7. For all new and converted Underground Injection Control wells a Cement Bond Log (CBL) is a required on the cased portions of the hole from the bottom of the casing to the top of the next shallower casing string for all casing strings other than the Surface Casing. Only a PDF, TIFF, or PDS visual image is required., LAS format is appreciated-not required.
8. Operator must provide all tops of formations encountered from surface to TD on the Form 5 when submitted.

| UIC | Form: (04) 401061766 06/16/2016 | Procedure is restricted to a total maximum injection volume of 80,000 barrels over a maximum of 10 days. |
| UIC | Form: (04) 401061766 06/16/2016 | Operator is required to contact COGCC and report results and/or progress of procedure on day 3 (post-injection of radioactive tracer), day 7, and day 10. |
| UIC | Form: (04) 401061766 06/16/2016 | Operator to review and if necessary adjust injection pressure used here at Puckett SWD H2 797 (API: 045-22631) with respect to recent pressures used for Hydraulic Fracturing job at Puckett SWD E1-797 (API: 045-22848). |
| UIC | Form: (04) 401161231 12/06/2016 | Operator is approved to increase maximum daily injection rate from 8,000 barrels per day up to 11,000 barrels per day through March 7, 2017 (approximately 12 weeks). After March 7, 2017 maximum daily injection rate is to return to 8,000 barrels per day. |
| UIC | Form: (04) 401161231 12/06/2016 | On or by July 15, 2017 operator is required to send COGCC a letter reporting any seismic activity that has occurred above a Magnitude 1.0 through July 1, 2017 in the area. Operator is to use the United States Geological Survey's Earthquake Hazards Program-Search Earthquake Catalog link as the basis of this report. |
| UIC | Form: (04) 401402233 09/14/2017 | Operator is given permission to temporarily increase Maximum Daily Injection Rate from 8,000 to 11,000 barrels per day from September 14, 2017 through October 31, 2017. |
