(1) Compliance with the most current revision of the northwest Colorado notification policy is required. See attached notice. (2) Compliance with the most current revision of the Garfield County Rulison field notice to operators is required, with the following exception: all field notice requirements specified in this notice to operators are superseded by the requirements of the most current revision of the northwest Colorado notification policy (see Condition Of Approval #1). See attached notice. (3) Compliance with the most current revision of the Mamm Creek Field Notice to Operators is required, with the following exception: all field notice requirements specified in this notice to operators are superseded by the requirements of the most current revision of the northwest Colorado notification policy (see Condition Of Approval #1). See attached notice. (4) Compliance with the notice to operators drilling wells in the Buzzard, Mamm Creek, and Rulison Fields, Garfield County and Mesa County is required. (Procedures and Submittal Requirements for Compliance with COGCC Orders Nos. 1-107, 139-56, 191-22, and 369-2 (July 8, 2010)). (5) Cement on production casing must be a minimum of 200' above the top of the Mesa Verde Formation (or 200' above the Ohio Creek Formation if present). Cement top verification by CBL is required. This requirement shall supersede the top of cement requirements in the Mamm Creek Field Notice to Operators.

1. Complete repair work or P&A well within 6 months (11/1/2024) from the date of identifying the failure 2. Operator will implement measures to control venting, to protect health, safety and environment, and to ensure that vapors and odors from well operations do not constitute a nuisance or hazard to public welfare.

1) For applicable operations , report work completed with a Form 4 Sundry REPORT OF WORK DONE - Repair Well noting final pressure test results and include any operational or job summaries. If the wellbore geometry changes or there is not an updated WBD in the well file submit an updated WBD. 2) Complete a bradenhead test within 30 days of completing the repair and submit results on a Form 17 3) Complete repair work or P&A well within 6 months (11/1/2024) from the date of identifying the failure 4) For applicable operations, submit a Form 5 or 5A (updated packer and tubing depths) within 30 days of the work with job related documents including a minimum of one of the following attachments: operations summary, cement job summary and/or wireline summary. Additionally, include CBL’s and any additional logs run.

WPX will run triple-combo open hole logs from well TD up to base of surface casing on one of the first wells drilled on a multi-well pad. Remaining wells on the pad will be logged with either cased hole pulsed neutron or triple-combo open hole. Every well will also have a CBL log from well TD up through well surface. Form 5 Completion Reports will identify wells on the pad with triple-combo open hole logs.

Minimization: TEP will make best efforts to minimize operations at these location during winter months by conducting operations, when possible, between 9:00am to 4:00pm when wildlife activity minimal.

Minimization: To minimize the potential for wildlife related traffic accidents, TEP has implemented speed restrictions for all lease roads and requires that all TEP employees and contractors adhere to these posted speed restrictions.
