(1) COMPLIANCE WITH THE MOST CURRENT REVISION OF THE NORTHWEST COLORADO NOTIFICATION POLICY IS REQUIRED. SEE ATTACHED NOTICE. (2) COMPLIANCE WITH THE MOST CURRENT REVISION OF THE GARFIELD COUNTY RULISON FIELD NOTICE TO OPERATORS IS REQUIRED, WITH THE FOLLOWING EXCEPTION: ALL FIELD NOTICE REQUIREMENTS SPECIFIED IN THIS NOTICE TO OPERATORS ARE SUPERSEDED BY THE REQUIREMENTS OF THE MOST CURRENT REVISION OF THE NORTHWEST COLORADO NOTIFICATION POLICY (SEE CONDITION OF APPROVAL #1). SEE ATTACHED NOTICE. (3) CEMENT ON PRODUCTION CASING MUST BE A MINIMUM OF 200' ABOVE THE TOP OF THE MESA VERDE FORMATION (OR 200' ABOVE THE OHIO CREEK FORMATION IF PRESENT). CEMENT TOP VERIFICATION BY CBL IS REQUIRED. (4) THIS PLANNED HORIZONTAL WELLBORE IS LOCATED APPROXIMATELY 100 FEET FROM AND PARALLEL TO ADJACENT HORIZONTAL WELLBORE API# 045-22051 (FEDERAL GM 702-4-HN1). OPERATOR HAS TEMPORARILY ABANDONED FEDERAL GM 702-4-HN1, AS DESCRIBED ON FORM 4 #400640800, AND THE WELL SHALL BE PERMANENTLY PLUGGED AND ABANDONED AFTER USE FOR PRESSURE MONITORING IS COMPLETE. (5) THIS PROPOSED HORIZONTAL WELL APPEARS TO PASS WITHIN 150 FEET OF THE WELLBORE PATH OF OFFSET HORIZONTAL WELL FED GM 702-4-HN1 (API#045-22051). Prior to drilling operations, Operator shall perform an anti-collision scan of the existing offset well. This anti-collision scan shall include a definitive MWD or gyroscopic survey of the offset well with included error of uncertainty per survey instrument, and compared against the proposed wellpath with its respective error of uncertainty. If current surveys do not exist for the offset well, Operator shall perform a gyroscopic survey to verify the wellbore path. The proposed well will only be drilled if the anti-collision scan results indicate that there is not a risk for collision, or harm to people or the environment. For the proposed well, upon conclusion of drilling operations, an as-constructed gyro survey will be submitted to COGCC with the Form 5.

Concerning the current pending permit you are working on (Doc. No. 400597974) the following can be inserted in the comment section. Concerning the existing wellbore for the GM 702-4-HN1, Federal regulators requested that, after P&A work performed between June 23-25, 2014 and reported in sundries to BLM and COGCC on July 11, 2014, the wellhead be left in place so gauges can monitor pressure during the completion operations of the new re-drill (GM 702RD-4-HN1). Even though the GM 702RD-4-HN1 and the original GM 702-4-HN1 will be drilled into the same objective formation (Niobrara), only the GM 702RD-4-HN1 wellbore will be producing from the Niobrara formation. Once successful monitoring has been completed from the original wellbore, the final P&A operations of the GM 702-4-HN1 will be completed and the GM 702RD-4-HN1 will then be put on-line.

Open hole resistivity and gamma logs shall be run to describe the stratigraphy of the entire well bore and to adequately verify the setting depth of surface casing and aquifer coverage. On a multi-well pad, these open hole logs are only required on one of the first wells drilled on the pad and the Drilling Completion Report - Form 5 for every well on the pad shall identify which well was logged.
