Daily report dated 9/28/2013 accepted as the Mechanical Integrity Test for this approval. However, if sidetrack does not commence by 4/30/2014, then a new TA Form 4 request will be required in addition to a formal MIT (charted or witnessed by COGCC Staff) and filed on a Form 21. This approval is not an approval to sidetrack. Operator must receive approval on a new Form 2 prior to milling a window in the casing and attempting a sidetrack of this well. |
