11/28/2022 | 1) At the conclusion of the 6 months from BH test date (3/23/23), submit a Form 4 BHP Sundry that summarizes current well condition. The sundry should include details of the future plans, flow information, number of times well was blown down, pressure data and a discussion of the abatement history for the previous 6 months. Please include the proposed pressure mitigation plan in the sundry.
2) Operator shall comply with COGCC Rule 419. Bradenhead Monitoring, Testing, and Reporting and Rule 420. Form 17, Bradenhead Test Report
3) Submit a current WBD on a Form 4 Sundry “COMPLIANCE with CONDITION OF APPROVAL (COA)” by 03/31/23
