06/15/2015 | 1) Provide 48 hour notice of plugging MIRU via electronic Form 42.
2) Properly abandon flowlines per Rule 1103. File electronic Form 42 when flowline abandonment is complete.
3) Revise procedure to provide annular cement to 200’ above the Mesaverde top and a casing stabilization plug (required at 3000’ intervals). Perforate at 3700’ if rate cannot be established through casing leak. Squeeze with a minimum of 40 sxs. Place at least 200’ of cement in the annulus and leave at least 100' in casing. Tag plug if not placed below a CICR.
