| Engineer | Form: (04) 403215299 11/22/2022 | 1) At the conclusion of the 6 months from BH test date (1/21/23), submit a Form 4 BHP Sundry that summarizes current well condition. The sundry should include details of the future plans, flow information, number of times well was blown down, pressure data and a discussion of the abatement history for the previous 6 months.
2) Operator shall comply with the Notice to Operators Drilling Wells in the Buzzard, Mamm Creek, and Rulison fields, Garfield County and Mesa County – Procedures and Submittal Requirements for Compliance with COGCC Order Nos. 1-107, 139-56, 191- 22, and 369-2 (“Bradenhead NTO”), dated July 8, 2010.
3) Operator shall comply with COGCC Rule 419. Bradenhead Monitoring, Testing, and Reporting and Rule 420. Form 17, Bradenhead Test Report
4) Submit a current WBD on a Form 4 Sundry “COMPLIANCE with CONDITION OF APPROVAL (COA)” by 02/31/23
