| Engineer | Form: (04) 403189276 11/30/2022 | 1) Operator shall comply with the Notice to Operators Drilling Wells in the Buzzard, Mamm Creek, and Rulison fields, Garfield County and Mesa County – Procedures and Submittal Requirements for Compliance with COGCC Order Nos. 1-107, 139-56, 191-22, and 369-2 (“Bradenhead NTO”), dated July 8, 2010.
2) Operator shall comply with COGCC Rule 419. Bradenhead Monitoring, Testing, and Reporting and Rule 420. Form 17, Bradenhead Test Report
3) At least once a year shut in bradenhead for 7 days or until the pressure reaches the bradenhead threshold and perform a bradenhead test. Report results on a Form 17, as specified in Rule 420 or other Director approved submittal method.
4) Comply with any CDPHE, Air Pollution Control Division rules or requirements for all atmospheric discharges.
5) Any liquids blown down are to be collected, stored, handled, and treated or disposed as E&P waste per COGCC’s 900 series rules.
6) At least one check valve is required for annular spaces that are tied to sales line or separator. Maintain equipment for pressure regulation and check valves in good working order.
7) At least one valve is required to monitor pressure and sample flow from the bradenhead.
8) Within 30 days of completing the work, submit a Form 4 Subsequent Report - Bradenhead Plan
-Include diagnostic information on causal factors for surface casing pressure.
9) Submit a current WBD on a Form 4 Sundry “COMPLIANCE with CONDITION OF APPROVAL (COA)” by 03/31/23
5) Complete installation of BH monitoring system by 4/31/2023 |
| Engineer | Form: (04) 404321619 08/27/2025 | 1. Operator will implement measures to control venting, to protect health and safety, and to ensure that vapors and odors from well operations do not constitute a nuisance or hazard to public welfare.
2. No later than one year from annual BH test date , conduct a new bradenhead test and submit the Form 17 within ten days of the test and submit a Form 4 Sundry that summarizes current well condition. The sundry should include details of the future plans, sample analysis interpretation, and the flow rate information and pressure data.
3. Operator will implement measures to get an estimate of the gas flow rate and/or liquid volume from the bradenhead. During the shut-in period record pressure data to adequately characterize the build-up.
4. If a liquid sample has not been collected within the last twelve months collect samples for laboratory analysis. Sampling will comply with Operator Guidance - Bradenhead Testing and Reporting Instructions, Appendix A: Liquid and Gas Sampling. Copies of all final laboratory analytical results will be provided to the ECMC within three months of collecting the samples.
