There is not an approved one in the COGCC well file. The operator is required by rule 311 to do so prior to plugging the well. When the work is completed, another COGCC form 6 – Well Abandonment Report-Subsequent Report of Abandonment complete with an as plugged WBD and supporting service company job tickets. |

08/21/2014 | 1) Plug added: provide balance plug in casing across squeezed HIC at 773'. Minimum 10 sxs from 823' to 723'.
2) Modify procedure as follows: perforate casing 100' below surface casing shoe (396') and place plug in annulus and casing from 396' to 246'. If a CICR is not used, tagging the plug is required. Tag plug at 246' at least 50' above surface casing shoe. |
