1) Provide electronic Form 42 Notice of MIRU 2 business days ahead of operations. 2) Prior to starting repair work a bradenhead test shall be performed. If the beginning pressure is greater than 25 psi, or if pressure remains at the conclusion of the test, or if any liquids were present contact COGCC Engineer for sampling requirements before pumping any cement. The Form 17 shall be submitted within 10 days of the test. 3) If cement is squeezed above the reported TOC, 3209', then the placed cement shall be verified with a CBL and documented with a Form 5 within 30 days of repair operations. For a cement squeeze in the cemented interval, document repair on Form 4 with supporting documentation. 4) After repair, MIT (10 day notice, contact inspector) and submit Form 21 in 30 days.

This oil and gas location is within CPW-mapped High Priority Habitat for Lesser Prairie Chicken Estimated Occupied Range. Well repair activities should not take place from March 1 to July 15. If site conditions warrant that well repair activities must occur between March 1 and July 15, the operator will consult with CPW to develop site specific avoidance, minimization, or mitigation measures prior to conducting any activities.

1) Submit Form 42 electronically to COGCC 2 business days prior to MIRU (spud notice) for the first well activity with a rig on the pad and provide 2 business day spud notice via Form 42 for all subsequent wells drilled on the pad. 2) Increased cement coverage from 3200-0' with Operator approval. Adjust the cement volume accordingly. Verify coverage with a cement bond log.

This well failed an MIT on 9/20/22 with a CA date of 3/30/2023 to repair or plug and is now subject to enforcement. The demonstrated lack of mechanical integrity makes this well ineligible for continued TA status or the inclusion on a 6A list. The corrective action date to have this well plugged or demonstrate mechanical integrity remains.

Due to the proximity to a mapped wetland, operator will use secondary containment for all tanks and other liquid containers. Operator will implement stormwater BMPs and erosion control measures to prevent sediment and stormwater runoff from entering the wetlands.

Plugging 1) Provide electronic Form 42 Notice of MIRU 2 business days ahead of operations and electronic Form 42 Notice of Plugging Operations 48 hours prior to mobilizing for plugging operations.   2) Plugs and squeezes will be placed as stated in the Plugging Procedure section of the approved NOIA unless revised by COA or prior approval from ECMC is obtained. 3) The wellbore must be static prior to placing cement plugs which are to be a minimum of 100' in length for all but surface plugs. Mechanical isolation requires a 25' cement plug, minimum. For plugs not specified to be tagged, a tag is required if circulation is not maintained while pumping plug and displacing to depth. Wait on cement(WOC) a minimum of 4 hrs before tagging a plug. Tag at tops specified or shallower. Notify ECMC Area Engineer before adding cement to previous plug due to low cement top. 4) Place a 50' plug (minimum) at the surface, both inside the inner most casing and in all annular spaces. Surface plugs shall be circulated to surface. Confirm cement to surface and complete isolation in all strings during cut and cap. After cut and prior to cap, verify isolation by either a 15 minute bubble test or 15 minute optical gas imaging recording. If there is indication of flow contact ECMC Engineering. Provide a statement on the 6SRA which method was used and what was observed. Retain records of final isolation test for 5 years. 5) With the Form 6 SRA operator must provide written documentation which positively affirms each COA has been addressed. 6) Operator must wait a sufficient time on all plugs to achieve the intended design. If at any time during the plugging there is evidence of previously unreported pressure or fluid migration, contact ECMC Area Engineer before continuing operations. 7) Plugging procedure has been modified as follows, Lansing D abandonment plug at 4080' set 1/1/21 to remain, Plug #1 - 3980', CIBP with 5 sx of cement, Plug #2 - 2930', perf and squeeze 45 sx into a CICR at 2880, upper perfs at 2800', sting out and pump 20 sx of cement to cover the upper perfs, WOC and monitor well for any fluid migration in the casing or the annulus before proceeding to next plug, notify ECMC Area Engineer of any previously unreported migration, Plug #3 - 1910', perf and squeeze 60 sx into a CICR at 1772', 10 sx on top, Plug #4 - 1200', perf and squeeze 60 sx into a CICR at 1150', 10 sx on top, Pressure test casing, CICR required for next plug if casing does not test, Plug #5 - 700', perf and circulate 220 sx of cement to the surface, Plug #6 - 50' of cement at the surface in both the casing and the annulus per COA #4, if needed.

Properly abandon flowlines per Rule 1105. If flowlines will be abandoned in place, include with the Form 27: pressure test results conducted in the prior 12 months as well as identification of any document numbers for a ECMC Spill/Release Report, Form 19, associated with the abandoned line.

Allow 30 days for Director review of the Form 27; include the Form 27 document number on the Form 44 for offsite flowline abandonment (if applicable) and on the Form 6 Subsequent.

Bradenhead Testing Prior to starting plugging operations a bradenhead test shall be performed if there has not been a reported bradenhead test within the 60 days immediately preceding the start of plugging operations. 1) If, before opening the bradenhead valve, the beginning pressure is greater than 25 psi, sampling is required. 2) If pressure remains at the conclusion of the test, or if any liquids were present during the test, sampling is required. The Form 17 shall be submitted within 10 days of the test. Sampling shall comply with Operator Guidance - Bradenhead Testing and Reporting Instructions. If samples are collected, copies of all final laboratory analytical results shall be provided to the ECMC within three (3) months of collecting the samples. If there is a need for sampling, contact ECMC engineering for verification of plugging procedure.
