6/20/2011 | 1) Provide 24 hour notice of MIRU to Jim Precup at 303-469-1902 or e-mail at james.precup@ state.co.us.
2) Set at least 570’ of surface casing per Rule 317A, cement to surface, and stage cement from 2420’ to 50’ above the surface casing shoe, verify coverage with CBL. Note: Proposed stage cement from 2500’ up into surface casing is acceptable.
3) If completed provide cement coverage from TD to a minimum of 200' above Niobrara. Verify coverage with cement bond log.
4) If well is a dry hole, set 80 sacks cement from 50’ below J-Sand base to 100’ above D-Sand top, 40 sks cement 50’ above Niobrara, 40 sks cement at 2420’ up, 40 sks cement from 1750’ up, 40 sks cement from 1250’ up, 50 sks cement ½ out, ½ in surface casing, 15 sks cement at top of surface casing, cut 4’ below GL, weld on plate, 5 sks cement in rat hole & 5 sks cement in mouse hole.
