1. Must conduct a bradenhead test no later than 11/03/2025. The Form 17 must be submitted within 10 days of the bradenhead test date. Email the area inspector and engineer minimum 10 days prior to conducting the bradenhead test to allow for a witnessed bradenhead test. Submit form 42-Other “Alternative MIT/Bradenhead test” minimum 10 days prior to conducting the bradenhead test. 2. If well is not producing by October 3, 2026, an official MIT will need to be performed and submitted on a Form 21 no later than 10/30/2026.

Open-hole Resistivity Log with Gamma Ray Log will be run from the kick-off point into the surface casing. A Cement Bond Log with Gamma-Ray will be run on production casing, or on intermediate casing if a production liner is run. The horizontal portion of the wellbore will be logged with a measured-while-drilling gamma-ray log. The Form 5, Completion Report, will list all logs run and have those logs attached.
