| Agency | Form: (02) 2007176 05/29/2009 | 1) NOTE SURFACE CASING SETTING DEPTH CHANGE FROM 1050' TO 1110'. INCREASE CEMENT COVERAGE ACCORDINGLY. 2) PROVIDE 24 HOUR NOTICE OF MIRU TO JIM PRECUP AT 303-469-1902 OR E-MAIL AT JAMES.PRECUP@STATE.CO.US. 3) COMPLY WITH RULE 317.I AND PROVIDE CEMENT COVERAGE FROM TD TO A MINIMUM OF 200' ABOVE NIOBRARA AND 200' ABOVE SUSSEX. VERIFY COVERAGE WITH CEMENT BOND LOG. RESTORE SURFACE LOCATION. |
| Engineer | Form: (02) 400136032 02/24/2011 | Prior to recompletion, operator must: 1) Provide 24 hour notice of MIRU to Jim Precup at 303-469-1902 or e-mail at jim.precup@state.co.us. 2) Monitor the bradenhead during well stimulation per Rule 341. 3) Upon well recompletion operator shall file a COGCC Form 5A, Interval Report reflecting the new perforations. |
| Permit | Form: (06) 401915883 01/28/2019 | Form 7, Monthly Report of Operations should be corrected back to January 2011 to reflect the recompletion of the Niobrara-Codell. |
| Engineer | Form: (06) 401915883 02/15/2019 | If there has not been a reported Bradenhead test within 60 days of plugging this well, prior to starting plugging operations a bradenhead test shall be performed. If the well was not sampled as part of a prior test it should be sampled if pressure is 25# or greater. If there is a need for sampling - contact COGCC engineering for verification of plugging procedures (Pressure greater than 25# or any liquids flowed to surface).
1) If, before opening the bradenhead valve, the beginning pressure is greater than 25 psi, sampling is required.
2) If pressure remains at the conclusion of the test, or if any liquids were present during the test, sampling is required.
Sampling shall comply with Operator Guidance - Bradenhead Testing and Reporting Instructions, Appendix A: Liquid and Gas Sampling.
The Form 17 shall be submitted within 10 days of the test.

| Engineer | Form: (06) 401915883 02/15/2019 | Operator shall implement measures to control venting, to protect health and safety, and to ensure that vapors and odors from well plugging operations do not constitute a nuisance or hazard to public welfare. |
| Engineer | Form: (06) 401915883 02/15/2019 | 1) Provide 48 hour notice of plugging MIRU via electronic Form 42.
2) Any flow shall be stopped prior to setting 1400' plug, contact COGCC for verification of plugging orders if flow present after setting 2500' plug.
3) Properly abandon flowlines as per Rule 1105. File electronic Form 42 once abandonment complete. Within 30 days of an operator completing abandonment requirements for an off-location flowline or crude oil transfer line the operator must submit a Flowline Report, Form 44.
