| HOW REMEDIATE: | Once source removal is complete, an estimated timeframe to achieve NFA status will be July 31, 2025. |

| INITIAL ACTION: | A third-party water hauler tank developed a hole resulting in 28 bbls of water to spill inside lined containment and 12 bbls of water to spill outside containment. The spill was contained entirely on pad and a hydrovac was dispatched immediately to begin the cleanup. |

| RECLAMATION PLAN: | The location will remain an active oil and gas facility. Reclamation will be in accordance with COGCC 1000 series rules. |

| SOURCE REMOVED: | The spill location has been blocked by a temporary lined containment for temporary frac tanks. The tanks and lined containment have since been removed. 1 waste characterization sample was collected and submitted for Table 915 analysis. Source removal will occur, and confirmation soil samples will be collected and submitted for Barium, SAR, and Boron, as previously approved on Form 19 Doc #403982260.
