| FURTHER SITE INV: | Confirmation samples will be collected following the removal of contaminated soils around SS2. |

| HOW REMEDIATE: | After TPH impacted soil removal, SAR, EC and HSB concentrations will be monitored for natural attenuation, utilizing seasonal precipitation events. Scout request approval of a reduced analyte list on all future samples consisting of these analytes only. |

| INITIAL ACTION: | On September 18, 2024 about 2:30 PM a low flowline pressure alarm led to the discovery of a riser leak on AC McLaughlin 65X. Produced injection water and oil were released to ground. All fluids stayed on well pad location. The well was shut in immediately following discovery the leak was contained on location. All proper notifications were made. Repair and clean up plans were also made. Spill area was washed with fresh clean water from the water plant. |

| RECLAMATION PLAN: | If reclamation is necessary, it will be completed following 1000 Series regulations. |

| SOURCE REMOVED: | TPH impacted soils will be removed from the area around SS2 to a depth of approximately two (2) feet BGS using hand tools and equipment. TPH impacted soil will be taken to Scout's Rangely Landfarm. |
