| HOW REMEDIATE: | Once source removal is complete an estimated timeframe to achieve NFA status will be November 30, 2024. |

| INITIAL ACTION: | A compromised fitting inside the separator cabinet resulted in 3 bbls of produced water to spill to the ground surface. |

| SOURCE REMOVED: | Limited source removal has occurred and approximately 15 cubic yards of impacted soil has been removed and hauled for disposal. A limited site investigation was conducted to delineate impacts around surface equipment. Additional source removal will occur and confirmation soil samples will be collected and submitted for TPH (C6-C36), Organic Compounds in Soils, and SAR as previously approved on doc # 403718967. |
