| HOW REMEDIATE: | Once source removal is complete the estimated timeframe to achieve no further action will be August 30, 2024. |

| INITIAL ACTION: | Following drilling activities, the drill rig mats were removed and stained soil was observed. It is anticipated that the impacted material is greater than 10 cubic yards. |

| RECLAMATION PLAN: | The location will remain an active oil and gas facility. Reclamation will be in accordance with COGCC 1000 series rules. |

| SOURCE REMOVED: | Limited source removal occurred and approximately cubic yards have been removed and hauled for disposal. Confirmation soil samples were collected and submitted for ECMC Table 915 analysis. Analytical results indicate elevated levels of pH, SAR, DRO and ORO. Soil Sample points SS08, SS13, SS14, and SS19 were collected at the surface near the wellheads on top of a cement barrier. The elevated levels of pH are determined to be naturally occurring as a result of high alkalinity associated with cement products. Additional source removal will occur at SS08, SS13, SS14, and SS19 and confirmation soil samples will be collected for TPH (C6-C36), and SAR. |
