HOW REMEDIATE: | Once source removal is complete an estimated timeframe to achieve a no further action will be April 30, 2024. |

INITIAL ACTION: | A compromised gasket on the Grinde 1-64-5-0108C separator resulted in fluid to be spilled to the ground surface. All spilled fluid remained on pad. A vac truck was dispatched to remove standing fluid and additional source removal was scheduled. |

RECLAMATION PLAN: | The location will remain an active oil and gas facility. Reclamation will be in accordance with COGCC 1000 series rules. |

SOURCE REMOVED: | Source removal occurred, and approximately 40 cubic yards were removed. Confirmation samples were collected and submitted for Table 915-1 analysis. Analytical results indicate elevated levels of SAR, pH, and EC. Additional source removal will occur, and confirmation samples will be collected for TPH (C6-C36), Organics, Boron, SAR, pH, and EC. |
