| FURTHER SITE INV: | A site investigation will be conducted pursuant to COGCC rule 911 to evaluate known historic impacts at the Rasmussen 28 Facilities (322889) oil and gas location pertaining to the removal of one produced water vault and ancillary piping. |

| INITIAL ACTION: | Automation trends indicate a loss of fluid from the produced water vault. The volume of impacts is currently unknown, but is determined to be beneath the produced water vault. A site invesitgation will be conducted to COGCC rule 911 at the Rasmussen 28 Facilities (322889) oil and gas location pertaining to the removal of the produced water vault and potential removal of production tanks and ancillary piping.
