INITIAL ACTION: | On December 16, 2022, historical soil impacts were discovered during reclamation activities at the former Miller 23-24 wellhead location. The COGCC issued Spill/Release Point ID 483772 for this release. On December 8, 2022, three (3) preliminary soil samples were collected from the reclamation area of interest, and submitted for laboratory analysis to determine if impacts were present. Based on field observations and PID readings, the soil sample exhibiting the highest degree of potential impacts (REC-SS01@3") was selected for waste characterization purposes and submitted for laboratory analysis of the full COGCC Table 915-1 analytical suite, using standard COGCC-approved methods appropriate for detecting the target analytes. Laboratory analytical results indicated that soil impacts were present due to polycyclic aromatic hydrocarbons (PAH), arsenic (As), selenium (Se), pH, specific conductivity (EC), and sodium adsorption ratio (SAR) results above the COGCC Table 915-1 soil standards and/or site-specific background levels (x 1.25 for metals). Groundwater has not been encountered during the site assessment activities conducted to date. Excavation and
site assessment activities are ongoing, and will be summarized in a forthcoming Form 27-Supplemental update. Soil sample location and field screening data are provided in Table 1. |

RECLAMATION PLAN: | Following the completion of excavation and assessment activities, the site will be restored to its pre-release grade, and the site will be reclaimed in accordance with COGCC 1000 Series Reclamation Rules. |

SOURCE REMOVED: | Excavation and site assessment activities are ongoing, and impacted soil will be transported off-site for disposal at a licensed disposal facility. Confirmation soil samples will be collected from the final excavation extents and submitted for laboratory analysis of the COGCC-approved reduced analyte list described herein. |
