| FURTHER SITE INV: | Further excavation is required. |

| HOW REMEDIATE: | Following the completion of excavation MarCom conducted field screening at the site and determined further excavation was required. Further excavation was completed and Groundwater samples were collected on 1/26/2022. After review of the analytical it was determined that additional excavation is needed. Additionally a plan for site specific at depth backgrounds has been submitted for approval. Impacts will continue to be excavated until confirmation samples are below COGCC table 915.1 limits. |

| INITIAL ACTION: | Historical Contamination was discovered at the site during flowline work on 11/8/2021. Following the discovery a Form 19 was submitted and excavation commenced to remove impacted soil. |

| RECLAMATION PLAN: | Following the collection of clean final confirmation samples, a form 27 will be submitted requesting backfill. Upon the completion of backfill the site will be restored, and KPK will consult with landowner for seeding. |

| SOURCE REMOVED: | The source using ex situ treatment with an excavator. Confirmation samples will be collected to indicate weather or not the source and impacts have been fully removed. |
