HOW REMEDIATE: | Additional potential impacts that meet the criteria outlined in Rule 912.b.(1) A. through J. will be reported to the Director in accordance with Rule 912.b.(1). A site-specific soil and/or groundwater remediation plan will be developed and submitted to the COGCC through a Form 27-Supplemental in accordance with Rule 912.c.(1). Field observations, screening results and applicable laboratory analytical results will be provided in the Form 27-Supplemental. E&P waste records of material transported off-site will be kept on file and available upon request. |

INITIAL ACTION: | This form has been prepared in accordance with COGCC Rule 911.a to provide prior notice of the removal and resetting of two separators and one produced water vessel at the Miracle 16 20-12 O SA production facility. The first (middle) separator was removed due to a reportable release from the produced water dump line below the separator. A Form 19-Initial Supplemental Spill/Release Report (COGCC Document No. 402833694) was submitted on October 7, 2021, and the COGCC issued Spill/Release Point ID 480896. The second (southernmost) separator will be removed to address impacted soils associated with the release. The dumplines will be removed to the extent necessary to facilitate remediation of the separator release.The produced water vessel will be removed to investigate suspected impacts below the vessel. Soil and groundwater sample location and field screening data are presented in Table 1. A topographic site map showing the geographic setting of the site location is provided as Figure 1. Excavation and assessment activities are ongoing and will be summarized in a forthcoming Form 27 Supplemental update. |

MONITORING PLAN: | Groundwater monitoring wells will be installed at the tank battery location to fully delineate the extent and magnitude of the remaining residual dissolved-phase groundwater impacts. The temporary groundwater monitoring wells will be sampled on a quarterly basis and submitted for laboratory analysis of Table 915-1 constituents. A groundwater monitoring location figure illustrating the locations of the surveyed temporary monitoring wells will be provided in a Form 27-Supplemental update. |

RECLAMATION PLAN: | The affected infrastructure will be replaced, pending completion of impacted soil excavation activities. The site will be reclaimed in accordance with COGCC 1000
Series Reclamation Rules, pending future facility decommissioning activities. |

SOURCE REMOVED: | Impacted soils will be removed and transported to a licensed disposal facility in accordance with Rules 905 and 906. Disposal records will be kept on file and available upon request. |
