| Leak Description: | We experienced a 2inch union leak on our gas leg of a three phase separator. Duration of the gas leak was 1.5 hours. Wells were shut in and the pressure dissipated to a point where we were able to extinguish the fire. |

| Root Cause Description: | We did pull insulation apart and found out that a lease operator had taken the piping apart about two weeks earlier and had issues putting the piping back together. He performed a pressure test and held but we believe based on the cold temperature (-14 deg F) that the pipe contracted enough to create the leak.

| Prevention Description: | Decision tree being developed to determine when more people need to be involved in maintenance work. Anytime piping has been disassembled, a pressure test will be performed along with a FLIR inspection before putting line back into service. |
