| Comment: | A water truck driver was connected to one of the load out bowls to conduct a water transfer to his truck. In addition to the truck drivers hoses, there was also 4 inch to 3 inch reducing assembly with camlock connectors. The driver checked his connections but failed to check the reducing fitting. Two of the cams on the reducing assembly were not tight. When the driver opened the load out bowl valve, water pressure caused the loose camlock fitting on the reducing assembly to come loose allowing produced water to flow out onto the ground. Further remediation activities proposed (attach separate sheet if needed): The area impacted by the release was field screened for hydrocarbon content. Field screening results indicated hydrocarbon contaminant levels below the COGCC Table 910-1 standard of 500 ppm TPH in soil. A sample was collected from the impacted area to confirm the field screening results. The sample was submitted to an accredited laboratory for analysis of the full Table 910-1 analytical suite for soils. If the analytical results are below Table 910-1 criteria no further remedial actions are planned. If the impacted area exceeds Table 910-1 standards, it will be remediated to levels which comply with Table 910-1. Soil generated by any remedial activities will be stockpiled on the WPX GM 413-34 well pad pending treatment or disposal options. AWAITING ANALYTICAL RESULTS
