if work proceeding under an approved Form 27: | N/A | | | | Spill Description: | | Tank level automation controls malfunctioned causing produced water and condensate to overfill a condensate storage tank. Once the maximum tank capacity was reached fluids spilled from tank into the lined metal secondary containment. Condensate was also pushed through the 4-inch vent line over to the combustor stacks resulting in a short lived fire event in two of the stacks. Contract personnel were on location when the malfunction occurred and were able to quickly stop the spill by shutting in the affected wells. The fluids feeding the combustor fire were also immediately stopped by shutting a valve on the vent line feeding the combustors. In addition, the 1-inch gas line supplying fuel gas for the combustor pilots was also closed. Once the site was secured, vac trucks responded and removed all fluids from the containment system. The containment is brand new and has no leaks. No fluids escaped from the secondary containment system. 100% cleanup of all fluids was achieved. | | |
