if work proceeding under an approved Form 27: | N/A | | | | Spill Description: | | Upon arrival on location, a lease operator detected gas coming from the area near the 15D-14 well's wellhead. The well and associated flowlline was isolated and the gas was verified immediately to be coming from the soil adjacent to the wellhead mentioned above. The failure point of the flowline was identified on 3/7/2023 via daylighting of the area where the gas was assumed to be coming from. The failure point is the first below ground ninety degree elbow where the flowline transitions at the wellhead from above ground to below ground. This ninety degree elbow and the next one downstream will be replaced. Once the repair is complete and it is safe to do so, the soil adjacent to the failure point will be assessed for impacts. | | |
