| Spill Description: |
| During routine testing activities at the Albert D Kurtz GU C True 1 wellhead, five shallow soil vapor points were installed in the vicinity of the
wellhead. On April 21, 2022, samples were collected from the five soil vapor points. The samples were submitted to Isotech
Laboratories for gas composition analysis. Sample results were received on June 22, 2022, and indicated the presence of a trace
concentration of thermogenic gas. Analytical results are attached. The topographic Site Location Map showing the geographic setting of
the release is provided as Figure 1. |
