if work proceeding under an approved Form 27: | N/A | | | | Spill Description: | | A water hauler forget to close a frozen loadout line valve after trying to pull water from an onsite above ground storage tank on 1/23/2022. The valve thawed out throughout the day and fluid conveyed through the valve and into the secondary containment. On the morning of 1/24/2022, a lease operator noticed through a review of automation trending that the tank was lower than normal and began to investigate why. Upon arrival on location, the lease operator discovered the above-mentioned valve opened and contents of the tank in the containment. Vacuum trucks were dispatched to the location to remove all accumulated fluid on 1/24/2022. On 1/25/2022, contract hydrovac operators began removing the gravel from the secondary containment. Once the gravel is removed, an inspection of the liner will be conducted. | | |
