The water hauler was unable to make it to the location and pull the load largely due to the significant snow accumulation from the previous storms. This condition went unreported to the operator. As a result operations continued and then on 11/12 the tank level continued to rise until it overflowed into the secondary containment. The containment held most of the fluid but was compromised and allowed a portion of the fluid to make it on to the working surface, but progressed no further. The tanks were shut in and OCC was notified. | | |
