if work proceeding under an approved Form 27: | N/A | | | | Spill Description: | | A leak was discovered in an elbow fitting on the flow line near the well head. Settlement of flow-line bedding / fill materials over time compromised the flow line and fitting which resulted in the leak. The calculated volume of the spill is approximately 12.5 bbls (produced water). The well was temporarily shut in while the defective portion of the flow line was replaced. Impacted soils have been excavated and will be land-farmed on-site. Soils from the excavation were field screened with Petro-Flag to determine extent of hydrocarbon impacted soils. A composite soil sample was collected and sent to an anyltical lab to confirm that COGCC 910-1 clean-up standards are met. Root cause of the spill is equipment failure due to settlement of flow-line bedding materials. There are no off-site, or other environmental impacts associated with this incident. Spill fluid did not left the pad; the area of impact was immediately below the defective flow line fitting. | | |
