| Comment: | NOTE: This is an updated Form 15. Original pit was shown perpendicular to the reserve pit. The Form 15 with a change of pit use to a Multiwell Pit, the pit is shown parallel to the reserve pit. The later is the correct orientation to the pit. The orignal pit was permitted was w/o sensative area determination; however,the multi well permit was was listed as being not in a sensative area based on the decision tree. This pit has been determined to be in a sensitive area. Hydrostatic testing has not been done. This pit is scheduled to be closed pending surface owner approval.
