NOTE: This is an updated Form 15. Original pit was shown perpendicular to the reserve pit. The Form 15 with a change of pit use to a Multiwell Pit, the pit is shown parallel to the reserve pit. The later is the correct orientation to the pit. The orignal pit was permitted was w/o sensative area determination; howver, the multi well permit was was listed as being not in a sensative area based on the decision tree. Hydrostatic testing has not been done. This pit is scheduled to be closed pending surface owner approval.
COA:
Operator will submit field verified demisions and capacitites to COGCC FORM 4.
Operator will cease use of pit or conduct a 72 hour hydrostatic test by December 31, 2011.
Operator will provide a status update or Form 27 by December 31, 2011.
Operator will provide analytical data for grab sample collected from pit via Form 4.
